For many women, one of the big priorities of their breast enhancement procedure is for the result to look natural such that anyone who didn’t previously know them, wouldn’t guess that they ever had surgery. To achieve the ever popular ‘natural’ look, Dr Miroshnik uses a variety of different techniques and couples them with the best suited implants to each individual. There is no one-size-fits all formula to achieve a natural look, as everyone’s body shape is different and unique in its own way. Dr Miroshnik is renowned for his ability to create as natural result as possible with his breast augmentation procedures. It may involve the use of both round or anatomic/teardrop implants of various widths and sizes.
The ‘enhanced look’ is popular in women who frequent beaches as it can look spectacular in a bikini. An emphasis is placed on providing maximum size and maximum upper fullness. In order to achieve these parameters, Dr Miroshnik will usually use what are known as high or extra high profile implants to achieve this look. The subtle look, which is shown here in one of Dr Miroshnik’s patients, is suitable for those who are starting with almost no breast tissue at all i.e. AA or A cup and are desiring a more ‘subtle’ result. The emphasis here is on shape rather than absolute size and implant sizes are kept on the lower side of the scale. It is a particularly popular choice in women who are petite or athletic who don’t want to be weighed down by their breasts. It is also the most commonly requested look by runway models and is commonly seen in women modelling for high fashion magazines like Vogue and Harper’s Bazaar. As sizes are kept subtle, it is a look that works well in clothes without a bra.
Some women who desire a breast augmentation procedure start with so little of their own tissue (AA -AAA cup), that their chest can be referred to as a ‘blank palette’! In these women it is imperative that extra care is taken for the implants to look natural and not look at all artificial or ‘stuck on’. Tuberous (also known as ‘snoopy’, ‘tubular’ or constricted) breasts are breasts that are misshaped, having grown in an unusual way due to the presence of a constricting ring of tissue at their base.The constriction leads to deficient horizontal +/- vertical development of the breast and may lead to areolar enlargement. Problems with breast augmentation may occur as a result of poor planning, poor surgery or simply as a result of the way scarring and healing has occurred in the individual patient.Dr Miroshnik treats many patients from all over the country who have had their breast augmentation surgery performed elsewhere. The assessment process and the technical aspects of the corrective surgery itself are usually many times more complicated than primary (or first-time) breast augmentation. The two very broad categories of implant shape are round and anatomic (teardrop), which are detailed below. Many women, who are considering breast augmentation surgery already have some sort of preference in mind when it comes to these shapes. It is important, however, to remember though that the way these shaped implants are used, and in particular their placement, will affect the way they look in any individual. The amount and quality of pre-existing breast tissue is also very important in the look that these two implant types can achieve.

The type of implant shown to the left in one of Dr Miroshnik’s patients, is a round , textured, high profile style.
Round Implants have a circular base and therefore a height and width that are equal. They also add volume equally to all parts of the breast. Round implants may be used by Dr Miroshnik to create a natural or enhanced look to the breast and the way they look is very dependent on how much muscle coverage is used during their placement.
Anatomic (or teardrop) implants have a shape that is made to look more like a real breast. They are more customisable than round implants as their width doesn’t have to equal their height. Anatomic implants may be used by Dr Miroshnik to create a natural or subtle look. They are also an excellent choice in those with very little breast tissue to start with, those with breasts that are somewhat droopy and those with tuberous or misshaped breasts.
During your consultation, Dr Miroshnik will suggest sizes that match the type of look you would like to achieve with your augmentation. Many women have a cup size in mind when considering breast enhancement, whereas others are happy to be guided by Dr Miroshnik.
The ideal sized implant is determined by considering the width of your chest, the amount of skin and quality of skin you have available as well as the look you would like to achieve.
Dr Miroshnik will use sizers to allow you to assess the look and feel of various implants that he may use. In some cases of asymmetry, two different sized implants are often the best way to produce an optimal result.
Today, the best breast implant manufacturers in the world supply a variety of silicone gel fills. All silicone gel inside these implants holds its shape to some extent like jelly (jello) or gummy bear lollies. However the gel itself does vary on how firm it is to the touch and its shape-holding qualities.
In general, the firmer a gel is the better shape holding characteristics it has, whereas the softer it is the less shape holding capacity it has.
You may see terms such as ‘memory shape’ , ‘memory gel’, or ‘highly cohesive gel’ used to describe implants that a filled with silicone gel that has high shape retaining properties. In fact, all of the best anatomical implants have a highly cohesive gel with maximum shape holding capacity.

All procedures are performed as day-only procedures using a fast recovery general anaesthetic. Most patients are back to office based duties within 7 days and are doing light exercises within the first 2 weeks.

Over time, factors such as age, genetics, pregnancy, weight changes, sun exposure, and gravity can cause the size and shape of the breast to change. Women who are dissatisfied with the size of their breasts, or have experienced changes in breast appearance can achieve a fuller, shapelier breast with breast augmentation. During breast augmentation, a breast implant is placed inside a pocket formed in the breast tissue. This can help to increase or balance the size of the breast, restore breast volume, or restore the shape of the breast after partial or total loss. It is important to realise that breast augmentation cannot correct significantly sagging or drooping breasts. In these instances, a breast lift is often necessary, which may be performed in conjunction with this procedure.
In this technique, Dr Miroshnik covers the implant with the entire pectoralis muscle for maximum coverage. The implant therefore lies under the entire muscle.
Here the chosen implant is placed in a pocket that Dr Miroshnik creates under the existing breast tissue. The implant therefore lies over the muscle and doesn’t interfere with it.
In this technique, Dr Miroshnik uses the lining of the pectoralis muscle (also called fascia) to cover the implant along with the breast itself. The implant still lies over the muscle and doesn’t interfere with it.
A combination of the above. Some of the implant in key areas is covered by muscle whereas other areas are covered by fascia/breast tissue. This technique is further subdivided into different ratios of coverage from 1 to 4.
Further developing the dual-plane method, Dr Miroshnik moves the muscle in such a way as to create volume in areas where it is most needed and hide volume where it is less needed. Customising the exact position of the pectoralis muscle in not only its vertical position but also its angulation allows for even more precise control of breast shape. Dr Miroshnik has pioneered this method of dual plane positioning.
Breast augmentation with an incision directly under the breast, along the inframammary fold (IMF), is by far the most common technique used by Dr Miroshnik for all breast enhancement and enlargement surgery. This is because the implant can be positioned most accurately into its pocket using this method and any form of implant can be used including custom-fitted anatomical, cohesive gel implants. The implant itself may be placed under the pectoralis muscle, under the breast tissue or a combination of both in the ‘dual plane’ technique. Indeed, the ‘dual plane’ technique of implant placement yields some of the best results possible in breast augmentation / breast enlargement surgery. This is because the implant can be positioned most accurately into its pocket using this method and any form of implant can be used including custom-fitted anatomical, cohesive gel implants. Silicone implants are made of a silicone rubber shell and are filled with silicone gel. There is no significant evidence that suggests silicone implants increase your risk for breast cancer, connective tissue disorders, or autoimmune disease. Modern silicone implants are far superior to saline implants in terms of feel, shape and longevity. It is for these reasons that silicone implants represent by far the majority of implants used for breast augmentation in Australia today.
The implants available differ in shape, texture, profile, width, height and volume. The type, style, and size of breast implants you choose are determined by your lifestyle, body contours, the amount of breast tissue you have, and the cup size and appearance that you would like to achieve. Dr Miroshnik will spend over an hour with you, on your first consultation, to determine which implant is best for you.
The breast consists of glands and milk ducts, surrounded by fatty tissue which provides its shape and soft form. The elasticity of your skin also contributes to its shape. As you know, certain factors such as age and pregnancy can affect the elasticity of your skin and breast tissue, which results in changes to shape and appearance.
Breast implants can be placed in two general locations, known as submuscular and subglandular placement. Submuscular placement refers to an implant that is placed partially or completely beneath the pectoralis muscle, against the chest wall. In contrast, subglandular placement refers to an implant that is placed beneath the breast tissue, but above the pectoralis muscle. Dr Miroshnik will help you determine the implant placement location that is best for you.
Breast augmentation/breast enlargement procedures typically last approximately 1 to 2 hours. Prior to the start of your procedure the treatment area will be thoroughly cleansed and an anaesthetic will be administered. Usually, this is a general anaesthetic, in which you are asleep for the entire procedure.
The inframammary incision is placed below the crease of the lower portion of the breast. A single, small incision is made along each breast. Using an instrument known as a retractor, Dr Miroshnik will open the incision in order to gain better access to the breast tissue below. Although the incision will be made as inconspicuously as possible, its length and appearance may vary depending on the type and size of implant and your body contours.
Dr Miroshnik will carefully separate the breast tissue with a minimally invasive cautery device and a scalpel, in order to reach the area of the breast in which the pocket will be formed. The pocket is then gently created to fit the chosen implant in the desired plane.

Dr Miroshnik will then insert the implants into their pockets and visually inspect your breasts to ensure that they are symmetric. This may involve sitting you up, while you are asleep, to see how the implants look under the influence of gravity.
If required, the pocket may then need to be adjusted to ensure the desired look is achieved.
The incisions will then be closed using sutures in the breast tissue. All sutures used are self-dissolving so there is no need to remove any of them during subsequent postoperative visits. The scars from the incisions will fade slowly over several months, but it may take up to a year for them to refine completely. As with any surgery, you will most likely experience some pain and swelling afterwards. The majority of the swelling will likely subside within a few days. However, some swelling may persist for several weeks.
Dr Miroshnik will place you in a special bra or compression garment to aid in the healing process. Compression garments provide support and comfort, minimize swelling, and help to maintain the position of the implants.
It is important to realise that your breasts will continue to change shape over time. However, the results from breast augmentation are typically long-lasting, which means that your breasts will maintain the fuller, shapelier appearance that you desire for years to come.
Breast enlargement using the areola, or peri-areola, technique involves an incision around the areola, which is the pigmented skin around the nipple. The incision is placed at the interface between the areola and the surrounding breast skin. Dr Miroshnik often uses this type of incision in women who have tuberous type breasts, women who are having some form of breast lift or women who are seeking concurrent areolar reshaping surgery.

Breast augmentation using the armpit technique, involves placing an incision in the hair-bearing armpit area. A pocket is then dissected from this location to place the implant into. This is the least used incision in Dr Miroshnik’s practice as it allows for very little flexibility, very little control of implant positioning and only a minimal ability to manipulate breast shape. It is also only suitable for round style implants.
